Responsibilities and Duties:
• Patient Assessment and Evaluation: Assess patients' physical abilities like movement, strength, and balance. Review medical history and tests to create personalized treatment plans. Use standard measures to track progress and adjust treatments.
• Treatment Planning and Implementation: Create custom treatment plans based on clinical findings and patient goals. Use various methods to improve mobility, reduce pain, and enhance function. Provide hands-on therapy to aid healing and improve movement.
• Patient Education and Counseling: Educate patients and families on their condition, treatments, and how to prevent further injury. Show proper techniques for exercises and daily activities to improve independence and safety. Encourage patients to be active in their recovery and take control of their health.
• Collaboration and Communication: Work closely with other healthcare professionals to ensure coordinated care and better outcomes. Communicate effectively with patients and their support network to address concerns and maintain continuity of care. Take part in meetings and planning sessions to provide holistic care and plan for patient discharge.
• Documentation and Record-Keeping: Keep accurate records of patient evaluations, treatments, and progress in electronic systems. Follow rules for documentation set by regulations and insurance companies to ensure proper reimbursement.
